Policy Violation Reporting Form

The Policy Violation Reporting Form is to submit concerns regarding organizations violating the GW Student Code of Conduct, the Health Promotion and Prevention Services ABCD Policy, or other behaviors which go against the values and expectations of the George Washington University.  This form is for non-hazing concerns or violations.  For hazing concerns, please visit the Hazing Reporting Form

To send a report, complete the fields below. If you choose to give your contact information, that information will remain confidential. However, if you choose to not submit contact information it will limit the ability for incidents to be investigated.  Your identity will not be disclosed to the individuals or groups in question.

By submitting this report, you certify that this information is accurate and truthful to your knowledge.

WARNING: False reports impede the ability of GW to meaningfully conduct investigations of legitimate incidents. Knowingly submitting a false report or misuse of this university form will result in disciplinary action under the Student Code of Conduct.
